What a name change declaration is
A name change declaration, sometimes called a name change affidavit, is a sworn statement that confirms your former name and your current name belong to the same person. It does not itself change your name on any register, a passport, Emirates ID or civil record is still updated through its own issuing authority. What the declaration does is bridge a gap, when a passport, visa, bank account, degree certificate and marriage certificate show slightly different versions of your name, a maiden name next to a married name, a transliteration spelt one way on one document and another way on the next, a surname added or dropped. Banks, universities, embassies and employers accept the declaration as the link between the two, so the rest of your paperwork is read as belonging to one person.
When a name change declaration is used
Spelling or transliteration difference
Links two spellings of the same name across passport, visa and bank records.
Addition or removal of a surname
Links a shorter and a longer version of the same name.
A general affidavit of name
A simple sworn statement that two names are one person.
What the declaration must contain
01
Your former name
Exactly as it appears on the older document.
02
Your current name
Exactly as it appears on your passport or Emirates ID.
03
The reason for the change
Marriage, correction, or another stated reason.
04
A declaration of truth
That the two names belong to the same person.
05
Your signature before the notary
Witnessed and recorded at the point of signing.
Matching every document
A name change declaration is read against the documents it is meant to reconcile, so it has to spell both names exactly as they appear, letter for letter, not as you would normally write them. One wrong character and the declaration adds a third version of your name instead of linking two. Send us copies of every document that shows a different spelling, your passport, visa, bank statements, certificates, so we can check the exact wording before you sign.

| Document | Fee | What you need |
|---|---|---|
| Name change declaration (affidavit) | AED 600 | Passport or Emirates ID, documents showing both names, originals and copies |
| Drafting around several documents | We quote after reading the document | Copies of every document showing a different name |
Fees may vary with the document; we confirm the figure before you sign.
What to bring
Passport and Emirates ID, and copies of every document that shows a different version of your name, visas, bank statements, certificates and the marriage certificate, where that is the reason for the change.
Declaration
Name change declaration
Links two versions of your name as the same person, for use alongside your existing documents.
Record correction
Correcting the record itself
Updating the name on a passport, Emirates ID or certificate through its own issuing authority.

Every character has to match
The declaration is only useful if it spells both names exactly as they appear on the documents it links. We check this against copies of each one before you sign.
- Both names copied letter for letter
- Every document you need linked listed
- Both language texts checked together
Who signs, and what we check
The person whose name changed
Passport or Emirates ID, original
Every document showing a different name
The marriage certificate, where relevant
A witness, where the receiving authority asks for one
Where a name change declaration gets stuck
A name change declaration is refused less often for its content than for a mismatch in the spelling itself, a middle name included on one document and dropped from the declaration, or a document the declaration should have linked that was never mentioned. We ask for every version of your name in use, not just the two you think matter, because a bank or university often holds a version you have forgotten about.
Common problems, and the fix
01
A middle name is inconsistent
Fix: state exactly how it appears on each document.
02
A document was left out
Fix: list every version of your name in current use.
03
The two languages differ
Fix: correct both texts together before signing.
04
The marriage certificate is not yet available
Fix: tell us, we can advise on the order to do things in.
After notarisation
01
Submit it alongside the mismatched documents
The notarised declaration is presented together with the documents it links.
02
Update the record where possible
Where a passport or Emirates ID can be reissued in the new name, do that too.
03
Keep a copy
Keep the notarised original and a copy for your own file.

A simpler version, or a different document
Where only one fact needs confirming, a general affidavit may be enough. Where the issue is an address, not a name, a different declaration applies.

Send copies of every document showing a different name. We check them before you sign.

Questions about name change declarations
Does the declaration change my name officially?
No. It confirms two names are the same person. Your passport, Emirates ID or certificate is updated separately, through its own authority.
What if more than two versions of my name exist?
List every version you know of. The declaration can link more than two where needed.
Can you draft the wording?
Yes. Send copies of the documents and we draft the exact wording before you sign.
Can I sign online?
Yes, our online notary service covers most name change declarations. Call or WhatsApp us to check yours.
Do I need this after marriage even if my passport already shows my married name?
Often yes, for older documents still in your maiden name, a degree certificate or an old bank account.
Is this the same as a name change affidavit?
Yes, the terms are used for the same document.
